APPLICATION FOR CHILD
CARE SUBSIDY
The Government Child
Subsidy is extended to all Singaporeans only. Parents applying for the
Government Subsidy will have the monthly fees subsidized depending on the type
of programme applied for and their eligibility. The Centre will apply for the
said subsidy on behalf of the parents. Mothers / Single fathers must inform
Centre when he/she ceases employment.

REGISTRATION
Parents
will have to complete and submit the Registration Form to Toddlers’ Cottage. In addition, photocopies of the following
documents are to be submitted together with the Registration Form:
1.
Child’s Birth Certificate
2.
Medical Record Book & Child’s Immunization Records for Measles, Triple
Antigen & BCG
3.
One recent Photograph of the child
4.
Parent’s Identity Card/Citizenship Certificates/Re-Entry Permit/Employment Pass
5.
Parent’s Certified Letter of Employment
6.
For a mother or single father who is self-employed, a statutory declaration is
required
Upon
confirmation of a place for your child, a registration fee of $100, deposit
equivalent to one month’s programme fee and the first month’s fee are payable,
in cash or via cheque payable to “Toddlers’ Cottage.
